SMQ Survey 2010

Thank you for your interest in the SMQ Survey 2010.

The MSSO will give away a gift - pop-up business card holder, digital photo keychain, or wireless mouse - to three randomly selected Survey responders. To be eligible for the gift, you must provide your email address in Section 1 of the Survey.

You are strongly encouraged to respond to the SMQ Survey online.

Alternatively, you may download the SMQ Survey as a Word document, fill it in, and email it to the MSSO Help Desk.

Survey responses must be completed by 16 April 2010. Your responses to the survey will help the MSSO continue to improve SMQs for all MedDRA users.
